The list of registered H K Practitioners is revised each year in October and you need to renew before the end of the month in order to remain on the listing.
The list appears on the web site (Find your nearest Practitioner) and is also sent to enquirers who ask for a printed version. At present the web site attracts well over 3500 visitors a month.

You can print an application / renewal form by selecting the image on the left. The form is in PDF format which means that you need Adobe Acrobat Reader in order to download and print it. It takes a while to download, so please be patient!
 Most computers have Acrobat Reader already installed, but you can download a free copy by clicking the Adobe icon:
HELP!! If you have any problems with downloading the PDF version of the form, please email me. I will email back a Word version of the form by return. However, Word documents do not always print as intended on some computer systems and the PDF version is the more reliable option. (PDF was designed for this very purpose and the formatting is independent of the operating system)

The annual rate for renewing your subscription to HK UK (including your listing on the Register of Practitioners) is as follows:

The application form should contain all the details you will need, but please remember that we need to see a photocopy of your insurance cover at the time you first join the listing. This is not necessary for later renewals.
